Registered Nurse
Position Overview:
As a Registered Nurse, you will provide skilled nursing care to patients in their homes on a scheduled basis. Your role involves executing physician-directed care plans under the guidance and supervision of the Director of Clinical Services or Clinical Manager.
Qualifications:
Must be a graduate of an accredited nursing program and hold a current California nursing license.
Preferably have a minimum of two (2) years of nursing experience.
Should align with the mission and values of our organization.
Capable of exercising independent judgment and initiative.

Licensed Vocational Nurse
Position Overview:
A Licensed Vocational Nurse will deliver skilled nursing care to patients in their homes, following the physician's care plan under the supervision of a Registered Nurse. All services are provided in line with the organization’s policies.
Qualifications:
Must have graduated from an accredited vocational nursing program and be licensed in California.
Preferably have at least one (1) year of nursing experience.
Should demonstrate alignment with our organization's mission and values.
Able to make independent decisions and exercise judgment.

Home Health Aide (HHA)
Position Overview:
A Home Health Aide provides essential personal care services to patients in their homes under the direction and supervision of a Registered Nurse. This role includes assisting with daily living activities and ensuring patient comfort and safety.
Qualifications:
Must meet the Medicare Home Health Aide training requirements and be certified as a Home Health Aide in California.
Should have a compassionate approach to caring for the sick and elderly.
Ability to follow instructions, and demonstrate reading and writing proficiency.
Maturity and the ability to handle the responsibilities of the position effectively.

Registered Physical Therapist (PT)
Position Overview:
As a Registered Physical Therapist, you will provide intermittent physical therapy to patients in their homes, following physician orders and care plans under the supervision of the Director of Clinical Services or Clinical Manager.
Qualifications:
Must have graduated from an accredited physical therapy program and hold a current California license.
Preferably have at least two (2) years of experience in the field.
Should be in agreement with the philosophy and objectives of our organization.
Capable of using independent judgment and initiative in patient care.

Medical Social Worker (MSW)
Position Overview:
A Medical Social Worker offers social work services to patients at home on an intermittent basis. This role is carried out in line with physician orders and care plans under the supervision of the Director of Clinical Services or Clinical Manager. Social work services may be provided directly or through a supervised assistant.
Qualifications:
Must hold a master’s or doctoral degree in Social Work from an accredited program and have one year of experience in a healthcare setting, or
Hold a bachelor’s degree in social work, psychology, sociology, or a related field with at least one year of experience in a healthcare setting.
Preferably have two (2) years of relevant experience (for assistants).
Preferably have one (1) year of home care experience.
Should align with the organization’s mission and values.
Ability to collaborate effectively with others to achieve common goals.

Speech Language Pathologist (SLP)
Position Overview:
A Speech Language Pathologist provides speech therapy services to patients in their homes on an intermittent basis. These services are provided in accordance with physician orders and care plans, under the supervision of the Director of Clinical Services or Clinical Manager.
Qualifications:
Must have a master’s or doctoral degree in Speech Language Pathology and be licensed in the state where services are provided, or
Have completed 350 hours of supervised clinical practicum, have nine months of supervised full-time SLP experience, and have passed a national exam.
Must be currently licensed in the state(s) where practicing.
Preferably have two (2) years of relevant experience.

Occupational Therapist (OT)
Position Overview
An Occupational Therapist provides intermittent occupational therapy services to patients in their homes, following physician orders and care plans under the direction and supervision of the Director of Clinical Services or Clinical Manager.
Qualifications:
Must be a graduate of an accredited Occupational Therapy program and/or be certified by passing the National Certification Examination.
Eligibility for the National Registration Examination by the American Occupational Therapy Association is required.
Must be licensed in the state(s) where practicing, if applicable.
Preferably have two (2) years of experience in the field.
