In close collaboration with each patient’s primary physician, we deliver a customized range of services including medication supervision, various forms of wound and ostomy care,restorative practices, simple pulmophysiotherapy, and extensive family and patient health education.
Our comprehensive care extends to managing diabetic needs, conducting lab work, administering intravenous infusions, and offering pre- and postoperative support. Additional coordination for necessary medical equipment ensures our patients’ utmost comfort and care continuity. Social Workers provide counseling for patients and their
loved ones in dealing with the social and emotional
aspects related to each specific illness. Our specialists
help family members to properly care for the patient by
communicating available community resources and
coping skills assessment. Social Workers play a vital role in our care team, offering essential counseling and emotional support to both patients and their loved ones as they navigate the challenges associated with illness. They provide guidance on dealing with the social and emotional aspects of specific medical conditions, helping patients and families cope with the stress, anxiety, and emotional upheaval that often accompany serious health issues. Certified Home Health Aides are an integral part of our care team, providing essential support to our patients in their day-to-day activities. They assist with personal care needs such as bathing, toileting, dressing, and meal preparation, ensuring that each patient maintains their dignity and comfort. Additionally, they help with mobility tasks, including walking and transferring, to promote safety and independence.
Beyond these fundamental duties, our Home Health Aides also focus on enhancing patients’ physical well-being through range of motion exercises and strength-building routines. They play a key role in creating a comfortable and supportive environment by taking care of light housekeeping tasks, which contributes to a healthier and more pleasant living space. Their compassionate approach ensures that patients not only receive the care they need but also feel valued and supported every step of the way.
A “shift care” refers to a period of in-home care provided to an individual through the Home and Community Based Alternatives (HCBA) waiver program, allowing them to receive necessary medical and personal care services in their own home instead of a nursing facility, typically including nursing care, personal assistance, medication management, and other support based on their specific needs, all delivered within a designated shift time frame.
