The Basics

Understanding home health.

Home health care brings skilled medical and therapy services into your home under a physician-directed plan of care — skilled nursing, physical, occupational and speech therapy, IV infusion, home health aides, and medical social work. It's designed to support recovery, manage medical conditions, and help you remain safely at home.

Home health focuses on recovery, rehabilitation, and managing medical conditions at home. Hospice focuses on comfort and quality of life for patients with serious or terminal illness. Both bring care into the home, but they serve different goals — our services are home health.

Home health includes skilled medical services — nursing, therapy, infusion — ordered by a physician and typically covered by insurance. Personal caregiving is non-medical help with daily living. Our certified home health aides provide personal-care support as part of a skilled, nurse-supervised plan of care.

It's ideal after a new diagnosis or surgery, following a hospital stay, with changes in medication or care needs, when specialized equipment is required, or when mobility and daily activities become challenging.

Anyone can start the conversation — a doctor, a hospital, a loved one, or you yourself. Call (818) 484-5730 or request care online. Home health services do require a physician-directed plan of care, but we handle that coordination: we'll work with your doctor to obtain orders and verify everything for you.

Patients typically qualify when a physician determines that skilled nursing or therapy services are needed at home. Eligibility depends on your medical condition, physician orders, and insurance coverage — call us and we'll help you find out quickly whether you qualify.

Reach out by phone or through our Request Care page. We verify your insurance and coordinate orders with your physician, a skilled nurse visits to build your personalized care plan, and your dedicated team begins visiting on a schedule designed around your recovery.

Discharge planners, case managers, and physicians can fax referrals to 747-286-2444, call (818) 484-5730, or use our referral page. Every referral is acknowledged and worked promptly — we coordinate orders, verify coverage, and schedule the start-of-care assessment without delay.

Coverage & Services

Insurance, services & what to expect.

Yes — Original Medicare covers eligible home health services in full, and most of our Medicare patients pay $0 out of pocket. We also accept Medi-Cal/Medicaid, select private insurance plans, and private pay, and we bill your insurance directly. Not sure about your plan? We'll verify your benefits at no cost.

Yes. Our RNs and LVNs provide hospital-quality care at home: medication management, wound and surgical-site care, vital-sign and chronic-condition monitoring, lab coordination, and health education for patients and families.

Yes — physical, occupational, and speech therapy, when ordered by a physician. Our therapists use advanced techniques, including VitalStim for swallowing recovery, to help patients rebuild strength, independence, and communication at home.

Yes. Our skilled nurses provide professional wound care as part of a physician-directed plan — dressing changes, healing-progress monitoring, infection prevention, and close coordination with your healthcare providers.

Yes — that's when many patients begin home health. We support recovery through skilled nursing, therapy, medication management, and coordinated care designed to reduce the risk of hospital readmission. Care begins promptly after referral.
