We are required by law to maintain the privacy and security of your protected health information (PHI), to provide you with notice of our legal duties and privacy practices, and to notify you in the event of a breach of unsecured PHI. Every member of our team is trained in HIPAA privacy and security requirements, and we maintain administrative, physical, and technical safeguards to protect your information.
Other uses and disclosures — including most marketing uses and any sale of PHI — are made only with your written authorization, which you may revoke at any time.
This page is a summary. Every patient receives our complete HIPAA Notice of Privacy Practices at the start of care, and a copy is available at any time upon request from our office.
